Job summary

Concord Hospital Health System is seeking a patient care coordinator to support patient registration, referrals, phone management, and other care coordination duties. You will act as the primary access point for patients and their families, ensuring timely, efficient, and compassionate service.

You will verify insurance data, assist with collections education, manage communications, and help route information between primary and specialty services while maintaining patient records.

Qualifications

• High School degree or GED required; annual registration competency exam may be required.
• Demonstrated customer service ability to interact with the public, staff and patients preferred.

Responsibilities

• Greets and arrives patients; secures signatures for all required documentation; follows CHMG policies and procedures.
• Obtains and verifies insurance and demographic information; makes collection attempts while educating on resources; reconciles work lists and reports.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information for new and existing patients; creates and maintains medical records.
• Processes and routes all incoming calls or requests; responds to inquiries.
• Schedules, confirms or reschedules patient appointments; promotes Patient Connect enrollment.
