Job summary

• Facilitate referrals, transfers, and escalations to specialized teams when necessary.
• Assist consumers in navigating complex healthcare systems and care pathways.
• Communicates and maintains relationships with Care Management team to promote proactive partnerships with patients.
• Monitor multiple conversations while maintaining high levels of accuracy and customer service.
• Maintain productivity, quality, and service-level expectations.
• Adhere to HIPAA, privacy, security, and organizational policies.
