Job summary

ChenMed LLC is hiring a Care Facilitator to be a friendly first contact for patients, greeting and checking them in and out. You will schedule appointments, update records, and ensure accurate data entry while maintaining patient confidentiality and a welcoming clinic environment.

The role requires strong customer service, familiarity with EMR systems, and the ability to navigate insurance details. Evening or weekend shifts may be needed, with travel limited to local area.

Qualifications

• Candidate must have strong customer service orientation and professional demeanor.
• Familiarity with medical insurance concepts and EMR systems is preferred.
• High school completion and relevant medical assistant training are required or preferred.

Responsibilities

• Greets and check-ins patients, schedules appointments, and handles messages accurately.
• Maintains lobby organization, records information, and ensures confidentiality.
• Acts as a gatekeeper for PCP and Specialist scheduling and coordinates follow-ups.
• Processes patient information, prints forms, and drafts New Patient packets.
• Collects copays and verifies insurance details, updating the system as needed.
• Provides friendly, responsive service and documents patient interactions in the EMR.
• Supports transportation coordination and assists with in-clinic logistics.
• Participates in team huddles and supports continuous improvement in patient experience.
