Job summary

Health Bridge is seeking a Patient Care Coordinator for a temporary 3-month assignment in Glendale, CA. The role supports appointment coordination, EMR data integrity, and patient communications.

The position runs Monday–Friday, 8:00 AM–5:00 PM, at an hourly rate of $18.42–$20.00. The coordinator will manage scheduling, data reporting, and HIPAA-compliant handling of patient information, while collaborating with dental teams to ensure timely preventive dental evaluations and accurate end-of-day

Qualifications

• High school diploma or equivalent.
• Excellent verbal and written communication skills.
• Strong attention to detail and time management.
• Proactive in identifying scheduling issues and providing real-time solutions.
• Proficiency with Microsoft Office and Google Suite.

Responsibilities

• Schedule and confirm patient appointments in the EMR.
• Identify patients eligible for Preventive Dental Evaluations per Health Bridge guidelines.
• Coordinate with in-office staff for check-ins and timely appointment adjustments.
• Record and monitor daily appointment metrics across schedules.
• Provide end-of-day summaries and activity logs to management.
• Serve as the primary contact for patient check-ins, cancellations, and reschedules.
• Communicate with clinical teams regarding Preventive Dental Evaluation appointments.
• Provide patients with results via phone, including translation as needed.
