Job summary

Community Health Association Inland Southern Region is seeking a Patient Care Coordinator to collaborate with chronically ill patients, families, and care teams to implement tailored care plans and improve access to services.

You will promote preventive care, reduce ER visits, and support patients in navigating the health system while connecting them to community resources and ensuring high-quality, patient-centered care.

Responsibilities

• Create and promote adherence to a care plan in coordination with patient, family/caregivers, and providers.
• Increase utilization of preventive care.
• Reduce emergency room visits and hospital admissions.
• Promote timely access to appropriate care.
• Improve continuity of care through relationships with tertiary providers and referrals.
• Support patient self-management and shared decision-making.
• Link patients to community resources to improve health and reduce costs.
• Work with patients one-on-one and provide guidance for complex issues.
• Assemble clinical background information and coordinate referrals.
• Review referral details with patients.
• Assist with problem solving of barriers to care (e.g., interpreters, transportation, prescriptions).
• Serve as system navigator and advocate for patient needs with carriers when needed.
• Identify and utilize cultural and community resources and build relationships with providers.
