Qualifications

• Basic Life Support preferred.

Responsibilities

• Coordinate safe, timely, effective, equitable patient care with the care team.
• Coordinate referrals and manage Plan of Care communications.
• Outreach to patients by phone or text to schedule with the care team.
• Maintain data on case management and outcomes.
• Support compliance with safety, infection control, and competencies.
