Job summary

NTACHC seeks a Care Coordinator to manage patient referrals, coordinate appointments with specialists, and verify insurance coverage. The role emphasizes accurate charting in the EHR and collaboration with clinic staff to provide quality patient guidance.

The coordinator will follow up to ensure referrals are completed, pre-authorizations for medications and DME are initiated, and all documentation is securely maintained in NextGen.

Qualifications

• High school diploma or GED required.
• Current Basic Life Support (BLS) certification recommended.
• Medical Assistant Certificate from recognized bodies recommended.

Responsibilities

• Provide referral details and arrange appointments for patients needing specialists or external visits.
• Verify patient insurance coverage with partnering institutions.
• Follow-up to ensure referrals and notes are obtained and scanned into the chart.
• Complete follow-up requests and manage tasks in NextGen by week end.
• Maintain HIPAA compliance in all charting and communications.
• Coordinate with clinics, specialists, physicians and management.
