SouthEast Alaska Regional Health Consortium is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, collaborating closely with the care team to ensure plans are followed and patient needs are met.

The role emphasizes coordinating referrals, communicating Plan of Care, and supporting community resources while maintaining high-quality patient interactions and data accuracy.

Qualifikationen

• 2 years of healthcare, behavioral health, dental, or relevant administrative experience required.
• Associate's degree or 2 years of college preferred, OR 2 years of relevant professional experience may be exchanged for a degree.
• Basic Life Support preferred.

Aufgaben

• Coordinate safe, timely, effective, efficient, equitable, patient-centered care.
• Assess self-management skills of patients and caregivers, and support wellness through coordination of services.
• Communicate Plan of Care with patients and caregivers.
• Manage and coordinate internal and external referrals.
• Serve as a communication portal between patients and their care team and handle lab results and forms.
• Perform patient outreach by phone or text to schedule with the care team.
