Job summary

SouthEast Alaska Regional Health Consortium (SEARHC) is seeking a care coordination professional to support patients with medical referrals, schedule appointments, and arrange travel and housing as needed.

The role emphasizes prompt communication, data entry, and collaboration across SEARHC and external agencies, with a focus on efficient coordination of patient care in a tribal health system.

Qualifications

• High School Diploma or equivalent required.
• 2 years working with the public in person and on the phone – preferred.
• 1 year working in the health care field OR 6 months education in a health care related field – preferred.
• Understanding of tribal health programs, referral process and alternate resources.
• Knowledge of data entry, retrieval and reporting.

Responsibilities

• Facilitates referrals for patients, including insurance authorizations for travel and appointments.
• Inputs data into databases and EHR for communication across the consortium.
• Schedules or facilitates appointment scheduling.
• Maintains patient referral information electronically.
• Coordinates travel for patients (air, ferry, lodging, ground transportation) in compliance with SEARHC policies.
• Other duties as assigned.
