The role includes assisting with referrals, coordinating communication between patients and the care team, and supporting data-driven care through SEARHC standards. Strong organizational and interpersonal skills are essential.

Qualifications

• 2 years of healthcare or relevant administrative experience.
• Associate’s degree or 2 years of college preferred in social work, social services, healthcare administration, business administration, or related field.

Responsabilités

• Coordinate patient care and referrals for timely, effective plans.
• Assist care team in coordinating safe, patient-centered care.
• Communicate plan of care (POC) with patients and caregivers; manage lab results and forms.
