SEARHC is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, closely collaborating with the care team to ensure treatment plans are followed and patient needs are met.

This role requires healthcare/administrative experience, strong communication skills, and proficiency with EHR systems and Microsoft Office. Travel is not required, with a focus on community-based coordination in Southeast Alaska.

Qualifications

• Associate degree or 2 years of college in related field (social work, healthcare admin, etc.).
• 2 years healthcare or administrative experience; can substitute degree.
• Bachelor’s degree may be exchanged for required experience.
• Basic Life Support preferred.
• 2 years healthcare/behavioral health/admin experience required.
• Internal candidates with 2 years in front/back office may be considered.

Responsabilités

• Assist care team in coordinating safe, timely, patient-centered care.
• Coordinate internal and external referrals.
• Communicate with patients and caregivers regarding Plan of Care.
• Perform patient outreach by telephone or text.
• Support care coordination across SEARHC system.
• Maintain clinic flow and data tracking.
• Ensure compliance with safety and competencies.
