Job summary

SouthEast Alaska Regional Health Consortium (SEARHC) in Sitka, Alaska is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, ensuring treatment plans are followed and patient needs are met. You will work closely with the care team to optimize outcomes and patient satisfaction.

The role requires 2 years of healthcare or related admin experience; an associate degree or equivalent is preferred.

Qualifications

• 2 years of healthcare, behavioral health, dental, or relevant administrative experience.
• Front or back-office experience may be considered for internal candidates.
• Associate degree or 2 years of college in a related field preferred.
• Bachelor’s degree may substitute for experience.
• Basic Life Support preferred.

Responsibilities

• Coordinate safe, timely, patient-centered care with the care team.
• Assess patient self-management skills and support wellness and autonomy.
• Communicate Plan of Care with patients and caregivers.
• Manage internal and external referrals.
• Serve as liaison between patients and care team and share lab results.
• Perform patient outreach by phone or text.
• Support care coordination across SEARHC system.
• Maintain clinic flow and track data.
• Advocate for patients and empower self-care.
• Identify community resources and partnerships.
• 50% referral management, 25% outreach, 25% care team communication.
• Other duties as assigned.
