Job summary

SouthEast Alaska Regional Health Consortium (SEARHC) is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient's care, collaborating with the care team to ensure plans are followed and patient needs are met.

This role emphasizes outreach, referral management, and communication with patients about plans of care, with an focus on coordinating services and maintaining efficient clinic flow in Southeast Alaska.

Qualifications

• Associate degree or 2 years of college in social work, social services, healthcare administration, business administration, or related field OR 2 years of relevant professional experience.
• A Bachelor’s degree in a relevant field may be exchanged for all required experience.
• Basic Life Support preferred.

Responsibilities

• Assists the care team in coordinating safe, timely, effective, efficient, equitable, patient-centered care.
• Communicates regularly with patients and caregivers regarding Plan of Care (POC).
• Responsible for management and coordination of internal and external referrals.
• Acts as a communication portal between patients and their care team, sharing lab results and completing forms as needed.
• Preformed patient outreach by telephone or text to schedule with the care team.
• Supports care coordination within and outside SEARHC to ensure a consistent system of care.
