Job summary

SEARHC is seeking a Patient Care Coordinator to manage and coordinate all aspects of a patient’s care, working closely with the care team to ensure treatment plans are followed and the patient’s needs are met.

Qualifications

• Associate degree or 2 years college in social work, social services, healthcare administration, or related field or 2 years relevant experience.
• Bachelor’s degree may substitute for experience.
• Basic Life Support preferred.

Responsibilities

• Assists care team in coordinating safe, timely, patient-centered care.
• Manages referrals and coordinates Plan of Care with patients and caregivers.
• Communicates with patients and caregivers about follow-up and outcomes.
• Maintains data on case management and care coordination.
