Atrium Health is seeking a Care Management Support role in Charlotte to assist patients with scheduling, registration, and access-related needs. You will respond to inbound chat inquiries professionally and empathetically, coordinating referrals and escalations as needed.

You will help patients navigate complex healthcare systems and care pathways while maintaining positive relationships with the Care Management team to enhance patient health outcomes.

Qualifications

• High School Diploma or GED required.
• Excellent verbal and written communication skills.
• Basic computer knowledge and ability to use Microsoft Office applications.
• Strong organizational skills.
• Effective interpersonal skills.
• Ability to respond quickly to changes in community and clinic settings.

Responsabilités

• Assist consumers with appointment scheduling, registration, and access-related needs.
• Respond to inbound consumer chat inquiries in a professional, empathetic, and efficient manner.
• Facilitate referrals, transfers, and escalations to specialized teams when necessary.
• Assist consumers in navigating complex healthcare systems and care pathways.
• Communicate and maintain relationships with Care Management team members to promote proactive patient partnerships.
• Monitor multiple conversations while maintaining high levels of accuracy and customer service.
