The Charlotte-Mecklenburg Hospital Authority, operating as Atrium Health Pineville, is seeking a Part-time RN Case Manager in Charlotte, NC. You will conduct assessments, develop discharge plans, coordinate services, and work with the care team to ensure timely transitions.

Requires RN license (BSN preferred), about 3 years of nursing and case management experience; schedule Monday-Friday 8:00am-4:30pm with occasional weekend coverage; pay range $38.20-$57.30 per hour.

Qualifications

• Must have RN license; BSN preferred.
• Typically requires 3 years of related nursing experience.
• Clinical experience within the assigned population and case management background preferred.
• Knowledge of Medicare A and B guidelines and managed care program implications.

Responsabilités

• Conducts complete assessments, establishes plans, and initiates interventions.
• Collaborates with patient, family and team to meet care goals and discharge plans.
• Initiates internal and external referrals to ensure timely progression of care.
• Documents discharge planning and utilization management activities.
• Communicates with healthcare team and collaborates with social work to coordinate care.
