Qualifications

• Serve as liaison between patients, providers, and other stakeholders.
• Coordinate patient care services, appointments, and follow-up plans with the team.
• Assist patients in understanding and adhering to their care plans.

• Educate patients and families about conditions, options, and self-care strategies.
• Empower patients to participate in healthcare decisions informed by their goals.
• Maintain accurate documentation of interactions, plans, and referrals.
