Job summary

SEARHC is seeking a Patient Care Coordination Specialist II to support patient flow, coordinate appointments, and manage registrations. The role requires accurate data entry in the EHR, effective communication with patients and providers, and collaboration with care teams to ensure timely access to services.

The position emphasizes multistakeholder coordination, attention to confidentiality, and alignment with SEARHC standards of service excellence. Shift: M-F, 8-5, full-time in Southeast Alaska.

Responsibilities

• Registers and pre-registers patients with accurate demographic, insurance, and eligibility information in the EHR.
• Performs patient check-in and supports self-check-in processes across multiple EHRs.
• Schedules, reschedules, and cancels appointments per protocols.
• Coordinates referrals and internal/external patient communications to support care teams.
• Verifies insurance eligibility, benefits, and authorization requirements.
• Maintains patient confidentiality and adheres to SEARHC standards.
