Job summary

University of Houston System in Houston, TX seeks a Care Coordinator to provide clinical and clerical support within the Tilman J. Fertitta Family College of Medicine.

You will communicate and document high-risk patient profiles, coordinate care, and support the care team to ensure continuity of care for all patients. You will handle triage calls, schedule pre-visit preparations, relay lab results, coordinate referrals, and assist with population health reports.

Qualifications

• Requires specialized training beyond high school level (up to 18 months).
• Minimum of three years directly related experience.
• Certification or registration related to the job is required.

Responsibilities

• Accepts patient calls and triages clinical inquiries for existing patients.
• Routes calls and documents notes in the EMR for provider instructions.
• Communicates lab results to patients in a timely manner with the care team.
• Reviews daily schedules to ensure pre-visit preparations are complete.
• Arranges timely delivery of pre-visit preparations with patients and providers.
• Refills medications per standing orders and initiates referrals as needed.
• Coordinates care for high-need patients with specialty or partner care.
• Assists in preparing reports to assess patient health needs.
• Performs other duties as assigned.
