Mercy Medical Center is seeking a Patient Navigator to support patient-centered care by coordinating referrals and scheduling with providers to ensure timely visits and services. The role acts as a clinic and hospital liaison, guiding patients through education and referral options while maintaining clear communication.

Ideal candidates have strong customer service experience, health insurance knowledge, and proficiency with electronic health records.

Qualifications

• Two years of customer service experience in a concierge, one-on-one environment.
• Experience educating the public on complex topics in easy-to-understand terms.
• Knowledge of health insurance terminology and terminology/abbreviations is preferred.

Responsibilities

• Coordinates appointments with providers to ensure timely delivery of office visits, diagnostic and treatment services.
• Works task list or referral work queue in the electronic health record.
• Maintains communication with patients and healthcare providers.
• Serves as a healthcare liaison to Mercy primary care patients, coordinating services inside and outside Mercy network.
• Obtains prior authorizations as needed for patients.
• Provides education and referral options to patients, emphasizing Mercy services.
• Communicates scheduling details (date, time, location, prep) to patients, primarily in person.
• Faxes patient orders to facilities prior to referrals.
• Places follow-up calls after referrals to patients.
