Job summary

University of California - Los Angeles Health is seeking a patient liaison to support physicians and patients in assigned primary and specialty care practices. You will act as a bridge between patients, physicians and hospital services, helping patients access care and navigate referrals.

The role requires strong prioritization, communication, and empathy, with responsibility for coordinating follow-up via telephone and Epic/Care Connect to ensure smooth care transitions for diverse patient

Qualifications

• Ability to set priorities reflecting the relative importance of duties.
• Strong interpersonal skills with Faculty, Staff, Administration and patients from diverse backgrounds.
• Ability to integrate competing demands into a productive working environment.
• Excellent verbal communication to convey complex problems clearly.
• Diplomacy and empathy with diverse patient populations.
• Willingness to provide coverage for Community Clinics as assigned.

Responsibilities

• Act as a liaison for patients and physicians in the practice.
• Assist patients in accessing services within the health system.
• Facilitate communication with referring physicians; refer patients to resources.
• Provide follow-up through telephone and EPIC/Care Connect.
• Support physicians and patients for assigned primary and/or specialty care physicians.
