Qualifications

• Requires specialized training beyond high school in a specific area or trade.
• Minimum of 3 years of directly job-related experience.
• National Medical Assistant Registration/Certification or Practical Nurse License may be required.

Responsibilities

• Accepts phone calls from patients forwarded by the front desk involving a clinical inquiry for existing patients.
• Performs protocol-based triage; routes phone calls as EMR notes to providers for review and instructions; returns calls to patients as directed by the provider.
• Places calls or creates letters regarding lab results for delivery to patients in a timely manner with coordinator.
• Reviews schedules daily to ensure pre-visit preparations are completed and results or reports are available in the medical record.
• Calls patients, consultants, imaging centers, or hospitals to arrange pre-visit preparations.
• Refills prescription medications per standing orders.
• Initiates external specialty or partner referrals to assist in continuum of care.
• Coordinates high-need patients to receive required specialty or partner care.
• Assists in completion of reports to assess and manage health needs of patient populations.
• Performs other job-related duties as assigned.
