Job summary

SEARHC in Wrangell, Alaska, seeks a Full-time Primary Care Coordinator to work with the care team to ensure treatment plans are followed and patient needs are met.

The role coordinates referrals, communicates Plan of Care with patients and caregivers, supports clinic flow, and maintains case-management data using SEARHC standards. Requires an associate degree or 2 years of college and 2 years of healthcare-related experience.

Qualifications

• Associate degree or 2 years of college preferred in social work, social services, healthcare administration, business administration, or related field.
• 2 years of relevant professional experience may be exchanged for a degree or coursework.
• Basic Life Support preferred.

Responsibilities

• Assists the care team in coordinating safe, timely, effective, patient-centered care.
• Communicates Plan of Care with patients and caregivers.
• Manages and coordinates internal and external referrals.
• Acts as a communication portal between patients and care team.
• Performs patient outreach by telephone or text to schedule with the care team.
• Supports clinic flow and maintains coordination data.
• Advocates for patients and caregivers at the service-delivery level.
