Qualifications

• Associate degree or 2 years of college in social work, healthcare administration, business administration, or related field (or 2 years of relevant experience).
• Bachelor’s degree may be exchanged for experience.
• Basic Life Support preferred.

Responsibilities

• Assist care team in coordinating safe, timely patient-centered care.
• Coordinate referrals and manage internal/external referral flow.
• Communicate Plan of Care with patients and caregivers on a regular basis.
• Support patient outreach and empower self-care through education.
