Job summary

You will collaborate with the Director of Clinical Quality to bridge gaps and promote patient-centered outcomes. In this role, you will advocate for patients, document care plans, and help navigate healthcare systems while maintaining regulatory compliance and high-quality standards.

Qualifications

• Bachelor's degree in Nursing, Social Work, Healthcare Administration, or related field.
• Proven experience in case management, care coordination, or a related healthcare role.
• Strong understanding of healthcare systems, community resources, and patient advocacy.
• Excellent communication, interpersonal, and organizational skills.
• Ability to work collaboratively with multidisciplinary teams.
• Compassionate approach with a focus on patient-centered care.
• Knowledge of healthcare regulations, privacy laws, and documentation standards.

Responsibilities

• Coordinate patient care across multiple specialties to ensure continuous treatment.
