Competenze

• High School Diploma or GED required.
• Associates degree in Business, Health Administration, or related field preferred.
• 1 year outpatient, ambulatory care, or community healthcare experience required.
• 2 years in outpatient clinical setting with PCPs; home care experience preferred.

• Collaborates with the primary physician and health care team to develop patient-centered goals and actions.
• Acts as patient advocate throughout the continuum and facilitates communication among patients, families, and clinicians.
• Coordinates follow-up care and ensures effective transitions across ambulatory, hospital, home, and skilled nursing settings.
• Performs care coordination, coaching, and home visits to support self-management and adherence.
• Documents encounters accurately and maintains up-to-date patient records.
