Job summary

Better Health Group is seeking a Health Coach to support patient outcomes through outreach, education, and care coordination. You will primarily contact patients by phone to facilitate transitions of care after hospitalizations and coordinate preventive care appointments.

Work closely with patients, providers, and internal teams to reduce avoidable readmissions, assist with medication adherence, document activities, and protect patient confidentiality under HIPAA.

Qualifications

• High school diploma required; degree preferred.
• 1+ year healthcare outreach or care coordination experience.
• Familiar with TCM, SDOH, and preventive care.
• Excellent communication and documentation skills.

Responsibilities

• Conduct proactive telephonic outreach to support engagement in care and follow-up activities.
• Coordinate Transitional Care Management after hospital discharges or events.
• Support reduction of readmissions by reinforcing discharge instructions and identifying barriers.
• Schedule Annual Wellness Visits and preventive care per protocols.
• Document outreach and follow-ups in designated systems.
• Collaborate with clinical teams to address patient concerns and ensure continuity of care.
• Maintain confidentiality per HIPAA and organizational policies.
• Perform additional duties as assigned.
