Qualifications

Responsibilities

• Care Coordination: coordinate appointments, tests, referrals; work with care teams.
• Patient Advocacy & Education: explain medical information to patients clearly.
• Transition Management: monitor patient transitions across settings; follow up post-discharge.
• Documentation & Reporting: maintain timely, confidential EHR documentation.
• Quality Gap Closure: contact patients to complete health maintenance and testing.
• Clinical Support: review clinical data and escalate issues when needed.
