Job summary

Qualifications

• High School Diploma or GED required.
• Associates degree in Business, Health Administration or related field preferred.
• 1 year of outpatient/ambulatory care experience required.
• 2 years of outpatient clinical experience with PCPs preferred.

Responsibilities

• Collaborates with the primary physician and care team to set patient-centered goals.
• Provides coaching and care coordination across ambulatory, hospital, home, and post-acute settings.
• Coordinates medication reconciliation, post-discharge needs, and follow-up care.
• Ensures clear, accurate documentation for every patient encounter.
• Maintains independent caseload management with attention to shifting priorities.
• Committed to safe handoffs and coordination with internal and external providers.
