Job summary

CentraCare Health is seeking a Patient Navigator to coordinate services and outreach for underserved patients in the community. This role acts as a liaison between patients and various healthcare providers, building trusting relationships to connect patients with appropriate resources within the local system.

The position focuses on removing barriers to access across multiple settings, working with a multidisciplinary team to efficiently navigate complex healthcare systems and improve timely

Qualifications

• High school diploma or equivalent required.
• 3–5 years of experience navigating complex healthcare systems required.
• Awareness of barriers for uninsured and underinsured patients preferred.
• Knowledge of local social services and community resources preferred.
• Experience with electronic health records (Epic preferred) to document patient encounters.

Responsibilities
