Aufgaben

• Expedites, initiates, participates, completes, tracks and updates the patient's plan of care.
• Communicates with primary care physicians, hospitals, specialists and post-acute care facilities to schedule appointments and identify gaps in care.
• Provides follow-up with participating providers to ensure patients follow through with scheduled appointments.
• Coordinates referrals and patient education resources and financial assistance as part of the care plan.
