Job summary

Concord Hospital in New Hampshire is seeking a Patient Care Coordinator who under supervision performs clerical and data management to support patient registration, referrals, phone management and care coordination. The PCC is the primary access point for patients and families, ensuring timely, efficient and compassionate customer service.

The role requires knowledge of medical office operations and experience with registration, scheduling and EMR systems, along with strong communication skills

Qualifications

• High School diploma or GED required; annual registration competency exam.
• Demonstrated customer service ability interacting with the public, staff and patients preferred.

Responsibilities

• Greet and seat patients; secure signatures for required documents; follow CHMG policies.
• Obtain and verify insurance and demographic information; make collections; reconcile lists.
• Facilitate patient care between primary, specialty and ancillary services.
• Welcome and coordinate intake of patient information; create and maintain medical records.
• Process and route all incoming calls or requests; respond to inquiries.
• Schedule, confirm or reschedule appointments; promote Patient Connect enrollment.
• Distribute mail, faxes and electronic documents; educate on form completion.
• Support a culture of "yes" in a patient-centered medical home.
• Work independently and in a team on projects and events.
