Are you passionate about delivering outstanding customer service and playing a critical role in patient care coordination? We are a growing, compassionate home health care agency seeking an energetic Patient Care Coordinator who thrives in a dual sales and scheduling role. If you're ready to drive growth while ensuring our clients receive the care they need, we want to hear from you!

What You'll Do:

  • Build strong referral relationships with community partners and healthcare providers
  • Drive new client acquisition and help expand our service footprint
  • Follow up on leads and close service agreements with prospective clients

Client & Caregiver Coordination:

  • Recruit, onboard, and maintain HR records for caregivers and direct care workers
  • Assist clients through the homecare service enrollment process
  • Schedule caregivers and oversee service delivery using Wellsky
  • Ensure Electronic Visit Verification (EVV) compliance and timely documentation

Customer Service & Support:

  • Communicate regularly with clients, caregivers, and care teams to support quality outcomes
  • Manage day-to-day operations and resolve issues proactively
  • Maintain a compassionate, professional tone with clients and their families

What We’re Looking For

Experience & Education:

  • Minimum of 2 years in home care, service coordination, or related fields
  • 1 year minimum sales experience
  • Prior experience with Wellsky preferred

Skills & Attributes:

  • Strong organizational skills and attention to detail
  • Excellent communication and public relations ability
  • Sales mindset — confidence in generating leads and closing opportunities
  • Strong computer literacy and typing skills
  • Proven ability to manage confidential information with integrity
