Job summary

You will assist the Prevention Case Manager with care coordination and follow-up, process and route patient records and referrals, ensure regulatory compliance, and deliver compassionate, high-quality customer service.

Qualifications

• Minimum of High School Diploma or GED.
• Bachelor’s degree in health or related field preferred.
• 2 years professional experience in a health or behavioral health practice preferred.

Responsibilities

• Schedule and monitor medical and behavioral health appointments and track patient progress.
• Support care coordination and follow-up with Prevention Case Manager.
• Process and manage patient records, referrals, and provider orders.
• Ensure regulatory compliance and deliver high-quality patient service.
• Collaborate with team and mentor colleagues as needed.
