CareSphere in Woodcliff Lake, NJ is seeking a Patient Care Coordinator to manage and coordinate all aspects of patient care, including onboarding, staffing, and case management. You will serve as the primary contact for assigned clients from case acceptance through long-term service retention.

The role requires strong organizational and communication skills, with a focus on proactive problem-solving and ensuring authorized care is consistently delivered.

Formación

• Experience in client services, healthcare coordination, or case management preferred.
• Familiarity with healthcare authorizations and staffing models is a plus.
• At least one year of experience in an office setting.

Responsabilidades

• Oversee onboarding documentation and admission agreements before services activate.
• Manage and optimize staffing schedules to fill all authorized service hours.
• Respond quickly to aide call-outs, resignations, or dissatisfaction to prevent disruptions.
• Recruit caregivers as needed to ensure coverage and prevent open shifts.
• Coordinate with vendors and third-party providers to meet client care needs.
• Maintain up-to-date client records with demographics, care plans, and incidents.
• Process authorization updates and adjust staffing accordingly.
• Manage discharges with proper documentation and timelines.
• Ensure caregiver shifts are correctly scheduled and reflected in the system.
• Monitor prebilling to verify visits for timely invoicing and payroll.
