Zaddy Solutions in Bay Shore, NY is seeking a Patient Care Coordinator to manage an assigned caseload of patients and ensure consistent, reliable home care services. You will communicate with patients, caregivers, MLTCs, and internal teams to maximize authorized-hour utilization and maintain thorough documentation.

The role requires strong communication and organizational skills, the ability to multitask in a fast-paced environment, and a proactive approach to resolving staffing and service

Responsibilities

• Manage an assigned caseload of patients and oversee day-to-day coordination of services.
• Ensure patients are staffed with reliable caregivers who meet schedule, needs and plan of care.
• Address staffing issues to prevent gaps in patient care.
• Monitor utilization and ensure authorized hours are staffed and used.
• Identify patients with low utilization and improve utilization.
• Maintain communication with patients and representatives about satisfaction and changes.
• Communicate with caregivers about schedules and patient needs.
• Coordinate with MLTCs and payors regarding authorizations and follow-ups.
• Monitor hospitalized or suspended patients and ensure follow-up.
• Restart services promptly with authorizations and updated schedules.
• Track patients with interruptions and follow status until resolution.
• Follow up on missed visits and uncovered hours promptly.
• Collaborate with staffing, intake, nursing, compliance, payroll to resolve issues.
• Complete required reports accurately and on time.
• Document communications timely and accurately.
• Escalate urgent issues to management when needed.
• Maintain accurate patient schedules and records in agency systems.
• Build strong relationships with patients and caregivers while staying professional.
