Qualifications

• Experience in intake, family communication and active case follow‑up.
• NJ-licensed LPN with coordination and client service skills.
• Nursing student with healthcare or care coordination experience.

Responsibilities

• Own day-to-day client experience from intake to ongoing case coordination.
• Coordinate care launches, staffing and start dates.
• Maintain accurate case notes and escalate issues promptly.
• Collaborate with RN for clinical input and care-plan adjustments.
