Qualifications

Responsibilities

• Manage an assigned caseload and coordinate services daily.
• Ensure patients have reliable caregivers meeting needs and plan of care.
• Address staffing issues to prevent gaps in care.
• Monitor utilization and ensure authorized hours are used.
• Communicate with patients, caregivers, MLTCs, and payors.
• Maintain schedules and accurate records in agency systems.
• Follow up on missed visits and service interruptions.
• Escalate urgent issues to management when needed.
• Complete required reports accurately and on time.
