Responsibilities

• Respond to phone calls with urgency and professional service.
• Gather initial information to open new cases and prepare clinical information packets.
• Maintain awareness of facility medical staff, services and programs.
• Locate resources and coordinate information for patient disposition and admissions.
• Track bed availability and coordinate transfers and transports.
• Provide updates on patient case status and bed placement requests.
• Assist scheduling and prioritizing discharge transports and rapid response to service requests.
