Job summary

Qualifications

• High School diploma or GED required; Pharmacy Tech cert or NYS LPN preferred.
• Three (3) years of pharmacy experience required.
• Ability to identify barriers to a successful care management path.
• Ability to interact with physicians and health-care team.
• Proficient PC/Windows skills including MS Office.
• Excellent problem-solving and time management skills.
• Excellent written, verbal and interpersonal communication skills.
• Ability to complete duties independently with flexibility in schedules.

Responsibilities

• Facilitate surveillance and educational outreach.
• Assemble/review clinical records from multiple sources.
• Forward patients out of acceptable range to a Pharmacist for review.
• Screen for issues using pharmacist-defined assessments; report findings.
• Initiate patient contact per protocol to gather accurate information.
• Document adverse events and tolerability for MD/pharmacy/management.
• Use CPR+, MS Office (Word/Excel/PowerPoint) to record data and reports.
• Assist in maintaining Prior Authorization forms.
• Maintain positive communication and resolve difficult situations.
