Job summary

Adobe Population Health in Phoenix, AZ, seeks a Medication Adherence Coordinator to support the clinical team with medication management, reconciliation, and adherence programs. This full-time, hourly hybrid position helps deliver population health initiatives across multiple states.

The MAC will collaborate with healthcare providers, patients, and the pharmacy team, collecting data and driving improvements in adherence and health outcomes. Travel to APH states is required.

Qualifications

• Minimum of 2 years of experience in a healthcare or population health setting.
• Experience with medication reconciliation and patient education is desirable.
• Home healthcare or related clinical setting experience preferred.
• Strong knowledge of medications, indications, dosages, and side effects.
• Proficiency with pharmacy software and EHR systems.

Responsibilities

• Support clinical team to improve patient outcomes through medication management and adherence.
• Conduct medication reconciliation and assist with population health programs.
• Identify barriers to adherence and develop intervention plans.
• Travel to healthcare facilities or patient homes across APH states.
• Maintain accurate records of patient and provider interactions.
• Collaborate with providers and pharmacy team to implement adherence strategies.
