Job summary

Personic Health seeks a Patient Intake Coordinator in Wilmington, DE to guide referrals into care. You will make initial outreach calls, collect demographic and insurance information, explain the home-based wound care model, and schedule the first visit.

The role requires clear, compassionate communication, strong data-entry skills, and collaboration with clinical and operations teams to ensure timely access and a seamless onboarding experience for patients.

Qualifications

• Strong communication skills and professional phone presence.
• High attention to detail and organizational abilities.
• Ability to manage multiple systems and workflows throughout the day.
• Familiarity with HIPAA and patient confidentiality requirements.

Responsibilities

• Conduct initial phone outreach to newly referred patients in a timely manner.
• Collect, document, and verify demographic, insurance, and clinical information.
• Explain Personic Health’s services, care model, and what patients can expect.
• Address patient questions with professionalism and empathy.
• Schedule initial visits and coordinate logistics with clinical/operational teams.
• Ensure accurate and complete data entry across required systems.
• Track and document referral status.
• Escalate clinical/operational concerns to supervisor as needed.
