Personic Health in Florida is seeking a Patient Intake Coordinator to help transition referred patients into care through outreach calls, data collection, and clear explanations of services and the care process.

You will schedule first visits, coordinate with clinical teams, ensure HIPAA-compliant data entry, and maintain compassionate, professional communication to support timely access to wound care at patients' homes or facilities.

Qualifications

• Strong verbal communication and professional phone presence.
• High attention to detail and organizational skills.
• Ability to learn and navigate EMR and other software systems.
• Experience in healthcare intake coordination or patient access preferred.

Responsibilities

• Conduct initial phone outreach to newly referred patients in a timely manner.
• Collect, document, and verify demographic, insurance, and clinical information.
• Clearly explain Personic Health’s services, care model, and what patients can expect.
• Address patient questions and concerns with professionalism and empathy.
• Schedule initial visits and coordinate logistics with clinical and operational teams.
• Ensure accurate and complete data entry across required systems.
• Track and document referral status.
• Communicate proactively with the RVP and support staff regarding barriers to scheduling or conversion.
• Escalate clinical or operational concerns appropriately.
• Maintain confidentiality and compliance with HIPAA regulations.
