Job summary

The Concord Hospital Health System is seeking a Patient Care Coordinator (PCC) to support patient registration, referral coordination, and reception duties under the Practice Administrator. This role emphasizes accurate data management, front-desk service, and timely communication.

You will be the primary access point for patients and families, coordinating between primary, specialty, and ancillary services, while promoting friendly, efficient, and compassionate care in a busy medical office.

Qualifications

• High school diploma or GED required; annual competency exam.
• Demonstrated customer service ability to interact with the public, staff and patients preferred.
• Familiarity with medical terminology preferred.

Responsibilities

• Greets and directs patients; secures signatures; follows CHMG policies.
• Obtains and verifies insurance and demographic information; educates on resources.
• Facilitates patient care between primary, specialty, and ancillary services.
• Welcomes and coordinates intake of patient information, and maintains medical records.
• Processes and routes all calls and responds to inquiries from departments and patients.
• Schedules, confirms or reschedules appointments; promotes Patient Connect enrollment.
• Distributes mail, faxes and electronic documents; educates on form completion.
• Supports a culture of “yes” in a patient-centered medical home.
• Works independently and in teams on projects and events.
