Job summary

The Salvation Army North & Central Illinois Division seeks a dedicated Coordinate Social Services Specialist to support the Pathway of Hope program. The role focuses on outreach, eligibility assessment, and delivering strength-based case management to families with minor children.

You will perform intake, develop and monitor case plans, arrange referrals, and collaborate with corps staff to connect clients with community resources while maintaining sensitive, confidential records.

Qualifications

• Education: Degree in human service area; prefer bachelor’s degree from an accredited college or university.
• Experience: Minimum of two years of case management experience in a comparable social service program that offers multi-faceted case management interventions.

Responsibilities

• Coordinate POH services and case management with corps staff and the Divisional POH Manager.
• Conduct outreach to social services sites to identify eligibility requirements and engage target populations.
• Maintain comprehensive case files, including assessments, case plans, progress notes, and discharge planning; ensure client confidentiality.
