Job summary

The Salvation Army is seeking a dedicated Case Manager to coordinate Pathway of Hope services in Kansas. You will perform outreach, screening, intake, and strength-based case management for families with minor children, aligning plans to housing stability and increased resources.

This full-time role requires collaboration with corps staff, community partners, and supervisors. High emphasis on confidentiality, documentation, and client advocacy, with potential weekend hours as needed.

Qualifications

• Experience in outreach and program intake processes.
• Ability to collaborate with diverse populations and staff.
• Strong written and verbal communication skills.

Responsibilities

• Coordinate POH services using strength-based case management.
• Conduct intake, assess eligibility, and develop action-oriented case plans.
• Maintain case files, document progress, and report outcomes to program leadership.
• Provide referrals and advocacy to connect participants with community resources.
