Job summary

The Salvation Army USA Central Territory is seeking a dedicated Pathway of Hope Coordinator to implement and oversee strength-based case management for families in need. Qualifications

• Bachelor's degree preferred; extensive experience may substitute for degree.
• Minimum of two years of case management experience in a social service program.
• Experience/outreach, ability to work with diverse populations in a culturally appropriate manner.
• Strong written and verbal communication skills; excellent interpersonal abilities.

Responsibilities

• Perform intake with required documentation for admission and referrals.
• Conduct a written assessment within 72 hours of contact and screen for safety/mental health issues.
• Develop a case plan within one week of intake with goals and objectives.
• Schedule regular meetings and conduct home visits as determined by the plan.
• Provide information and referrals to community resources; advocate as needed.
• Maintain comprehensive case notes and files; document progress and discharge plans.
• Coordinate case management with staff and contracted services.
• Track unmet needs and report outcomes to POH program management.
