Job summary

The Salvation Army is seeking a Case Manager for the Pathway of Hope program in Northland. You will coordinate social services, conduct intakes, develop case plans, and provide referrals to help families achieve housing stability and financial empowerment.

Responsibilities include home visits, budgeting classes, documentation, advocacy, and collaboration with staff and community partners to support participants’ goals and outcomes.

Qualifications

• Education: Degree in human service area; prefer bachelor’s degree from an accredited college or university.
• Experience: Minimum of two years of case management experience in a comparable social service program that offers multi-faceted case management interventions designed to address the needs of families utilizing a strength-based approach.
• Skills/Abilities: Outreach, collaboration, English proficiency, and ability to work with diverse populations.

Responsibilities

• Coordinate social services activities for the corps and provide direct assistance or referrals based on criteria.
• Conduct intake and assess participants for POH eligibility and develop a case plan with goals and timelines.
• Perform regular home visits, life skills and budgeting classes, and maintain detailed case notes.
• Provide advocacy, information and referral services, and coordinate with staff and partners.
• Maintain client confidentiality and track program metrics and outcomes.
• Attend meetings and represent the agency in the community while promoting the POH program.
