Job summary

The Salvation Army North & Central Illinois Division is seeking a dedicated professional to coordinate Pathway of Hope services. You will conduct outreach, assess eligibility, and provide strength-based case management to families with minor children, aiming to stabilize housing and income while promoting self-determination.

Responsibilities include intake, developing and updating case plans, home visits, referrals, and coordinating with staff.

Qualifications

• Bachelor’s degree in a human service field or equivalent experience.
• Minimum of two years of case management experience in a social service program.
• Ability to speak and understand Spanish and English.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Coordinate social services activities for the corps.
• Provide direct assistance and/or referral to clients in need based on established criteria.
• Assess families with minor children for POH eligibility and deliver POH services using strength-based case management.
• Develop a case plan with participants addressing housing stability, income/resources, and self-determination.
• Maintain case files and comprehensive notes, documentation of progress, discharge plan and follow-up.
