Job summary

The Salvation Army is seeking a dedicated Pathway of Hope (POH) Coordinator to lead outreach, intake, and strength-based case management for families with minor children in the Dodge City area.

You will collaborate with corps staff, provide referrals, develop discharge plans, conduct home visits, and maintain records in SIMS while promoting housing stability, income growth, and self-determination for participants.

Qualifications

• Education: Degree in human service area; prefer bachelor’s degree from an accredited college or university. Extensive experience in strength-based case management may substitute for bachelor’s degree.
• Minimum of two years of case management experience in a comparable social service program.
• Experience and/or strong interest in community outreach, organization and community capacity development.
• Ability to work with people from diverse racial, ethnic, and socio-economic backgrounds; collaborative skills.

Responsibilities

• Coordinate POH case management for families; outreach to identify eligibility; provide referrals and advocacy.
• Perform intake, assess needs within 72 hours, and develop case plans with goals for housing stability and increased income.
• Schedule regular meetings, conduct home visits, and connect participants to community resources.
• Maintain case files and SIMS records; ensure confidentiality and accurate documentation.
• Participate in program evaluation, reporting, and coordination with staff and partner agencies.
